Case Management

LEAP offers case management services to eligible individuals who are diagnosed with an intellectual disability and/or autism spectrum disorder.

Our case managers help adults connect to services and supports, navigate the adult developmental services system, advocate for needs/services, and monitor the plan and progress toward goals.

LEAP also offers representative payee services to people served in our case management program.
